The American Board of Allergy and Immunology-A Conjoint Board of the American
Board of Internal Medicine and the American Board of Pediatrics-is the ONLY Medical Specialty Board that Certifies individuals
to be trained experts in the field of allergy and immunology and related diseases. To become Board Certified, a physician
must first be Board Certified by the American Board of Internal Medicine or the American Board of Pediatrics. The physician
must then spend two years in an approved training Fellowship in pediatric and adult Allergy and Immunology, and then pass
the Examination in Allergy and Immunology-a training period totaling at least five years and two Board Certifications.

graduate medical education was completed in Pediatrics at Children's Hospital of Michigan in Detroit, Michigan. He completed
a two year Fellowship in Allergy and Immunology at Henry Ford Hospital in Detroit, Michigan. Dr. Fordyce sees both pediatric
and adult patients.

Diane L. Baranowski, M.D. is Board Certified in Allergy and Immunology. Her initial
post graduate medical education was completed in Internal Medicine at William Beaumont Hospital in Royal Oak, Michigan. She
completed a two year Fellowship in Allergy and Immunology at Henry Ford Hospital in Detroit, Michigan. Dr. Baranowski sees
both pediatric and adult patients.

The answering service will then forward your message to the physician on call. PLEASE CALL DURING REGULAR OFFICE HOURS
FOR PRESCRIPTION REFILLS. In case of severe health problems that cannot be handled by phone, go directly to Oakwood Hospital
Dearborn Emergency room or the the nearest emergency room. For extreme emergencies, dial 911.
